Mandated Reporter Submission Page

All trainees who see clients in the GAP clinic are required to complete the two-hour online NYS Mandated Reporter Training. The training is free and can be accessed through this link. Use the form on this page to upload your certificate of completion from New York State Office of Children and Family Services.
